Fault exceptions work a little differently than other exceptions. Since faults are reported continuously, the GO Device will only create a fault record once every ignition cycle. So when a vehicle is turned on and there is a fault, a single record will be sent. When the vehicle is turned off and on again then, if the fault is still present then another fault record will be sent.
The first fault record will cause an exception to be created. The next one will just cause the exception to continue. The exception will end when the Go Device detects an ignition cycle and no fault.
